  • Usual address.
  • The doctor who treated the person during their last illness will usually issue a medical certificate of the cause of death. Take this to the Registrar. Bring the medical card, birth certificate and passport if you can.
  • If the deceased got a pension or State Benefits of any sort, inform the Hammersmith & Fulham Registrar.

What certificates will be issued when you register a death in Hammersmith & Fulham?

  • Death certificate – buy several as they are much cheaper on the day, and you will almost always need more than you expect.
